By registering for participation in the Girls Basketball program, you agree to the following terms and conditions: * Assumption of Risk: I understand that participation in sports activities involves inherent risks of physical injury, and I voluntarily assume all risks associated with my participation. * Medical Authorization: In the event of injury or illness during participation in the program, I authorize program staff to obtain necessary medical treatment for myself or my child. * Release of Liability: I release, waive, and discharge the program organizers, coaches, staff, and volunteers from any and all liability for injuries, damages, or losses that may arise from participation in the program. * Consent to Photograph/Film: I consent to the use of photographs, videos, or other media recordings of myself or my child for promotional purposes related to the Girls Basketball program. * Code of Conduct: I agree to abide by the program's code of conduct, which includes respecting coaches, teammates, opponents, and officials, and maintaining good sportsmanship at all times. * Compliance with Rules and Regulations: I agree to comply with all rules, regulations, and guidelines established by the program organizers, coaches, and facility staff. * Payment: I understand that registration fees are non-refundable unless otherwise specified by the program organizers. * Parent/Guardian Consent (if applicable): As a parent or legal guardian, I consent to my child's participation in the program and agree to the terms outlined above on their behalf.
